New Patient Information

Welcome to the Matamata area!

For new patients enrolling at Matamata Medical Centre, enrolments are open to people residing in the immediate Matamata Area.

When you move into the area, it is important to enrol early, even if you don’t require a doctor’s appointment right away.  Visitor fees do apply if you are not enrolled at our practice.

Please note that not all of our doctors are able to take on new patients and we are unable to hold waiting lists.

What you will need

To enrol, all patients will need to come into the medical centre. Everyone over the age of 16 will need to complete their own form.  If you would like to pre-complete the form, you can download it below and print it out to complete. Please note that we cannot accept this via email.

Please bring with you, ID for everyone enrolling (passport, driver’s licence, birth certificate, student ID) and proof of your current address (e.g. a utility bill, lease agreement, etc). For patients under 16 years’ we require parent/guardian information (including proof of custody if applicable).

For people currently on work visas, you will need to hold a two-year visa to qualify to enrol. We will also need to sight and verify your visa to ensure your eligibility to enrol.
